Affidavit & Commissioner of Oaths Authentication

Have a legal declaration or statement of facts prepared, and authenticated before a commissioner of oaths. Commonly used for name changes, change of birth date, and other affidavits required to accompany official documents. Document will be delivered electronically, and pickup of the physical document may be arranged with our office upon completion.

Type of Affidavits:

1. Correction of Name: affidavit to correct name or to reflect additional names
2. Correction of Birth Date: affidavit to correct date of birth
3. Sponsorship: affidavit to study abroad, for traveling abroad, etc.
4. Consent: affidavit to confirm address, citizenship, single parents, etc.
